Please let us know if you will be attending the Banquet. Each Player, Coach, Manager will receive a free ticket for themselves. Anyone attending the banquet with their player will need a ticket. Once this form is completed an invoice will be sent to you with the number of additional tickets needed so payment can be made. Once the invoice is paid the tickets will be mailed to you. The deadline to RSVP for tickets is 11/16/2018. After this time no additional tickets will be sold.
